Care Management

 

Who’s who and what do we do?

Within the wider LDU we have a small dedicated team responsible for the planning, delivery and coordination of the Care Management Training Programme.  Care Management training is focussed on areas that have an impact on the day to day practice of adult social care practitioners employed by Northumbria Healthcare Community Business Unit.  The Care Management staffing group includes social workers, care co-ordinators and other practitioners involved with the delivery of support and care to a range of individuals with specific needs.  

The Care Management Training Team is made up of a number of people each with a specific area of responsibility.  They are:

 

Tracey Horseman – Staff Development Officer

Tracey leads our small specialist Team in the successful delivery of the Care Management/Care Co-ordination Training Programme and also has responsibility for the successful placement of all social work students within the organisation. Tracey takes a lead role and directly facilitates a number of related training courses.

 

Leigh Waller – Training Practitioner

Leigh is responsible for the Safeguarding Adults Training Programme and other related subjects including Mental Capacity Act.  Leigh also liases closely with the Safeguarding Adults Unit in Northumberland and takes an active role in the development and coordination of the Safeguarding Adults Training Strategy across all sectors.  Leigh works closely with the Private, Voluntary and Independent sector in developing specialist training to meet service needs.

 

 

Ceinwen Haydon – Training Practitioner

Ceinwen is responsible for the planning, coordination and delivery of Mental Health associated training and various other topics including – DoLs, MCA, MAPPA, MARAC and the Physical Conditions Programme.  Ceinwen also supports Social Work Students and Newly Qualified Social Workers in their practice development.

 

 

Anne Lambert – Learning & Development Officer

Anne is responsible for coordination for the Northumberland Safeguarding Children’s Board Training. Anne delivers training on a range of Safeguarding issues.
